blefaroplastika-1Blepharoplasty (from Greek biepharon – eyelid) or eyelid plastic is the surgical eyelids correction enabling to change eye form, to eliminate various eyelid handicaps, including wrinkles and eyelid hernias, as well as eliminate or reduce aging changes around eyes. Blepharoplasty can be performed independently or together with other cosmetic face surgeries.

By means of blepharoplasty you can eliminate signs of aging especially occurred in palpebral area and to change weary sight.

The most common defects of palpebral area are the following:

  • heavy, falling superior eyelid, with redundant skin forming fairly appreciable fold;
  • inferior eyelid flagging and aging;
  • evagination of “fatty tissue” in the area of superior and inferior eyelid, well-known as “fatty baggy lower eyelids”. Fatty hernia leading to baggy lower eyelids, as well as redundant skin and its edema in the area of superior eyelids;
  • numerous horizontal wrinkles due to skin expansion;
  • inconvenience during make-up and eye fatigue – redundant skin of superior eyelids preventing normal view that is usually worsening toward evening.

Blepharoplasty does not eliminate such defects as small fine lines in the infraorbital region and wrinkles in the area of lateral angle of eyes, they require other surgeries. Blepharoplasty does not eliminate wrinkle between eyebrows, “crow’s feet”, rims, hollow eyes. Eyes are the aesthetic center of the face; therefore, blepharoplasty is also used for improvement of appearance as a whole and elimination of signs of aging in this area. Blepharoplasty forms young, vigorous and rested face appearance. Today blepharoplasty is one of the most common face surgeries.

blefaroplastika-2We first heard of attempts of surgical eyelids form correction in the early 20-th century: in 1901 – Dr. Hollander has performed the first surgery on elimination of superior eyelids overhang having combined it with forehead “lifting”, and in 1906 – K. Miller has executed isolated blepharoplasty by simple excision of redundant skin. In the following decades methods of surgery are continuously improved – now surgeons aim not so much to trim a skin as to remove fatty tissue redundancies with single-stage plastic reconstruction of muscles surrounding palpebral fissure.

Blepharoplasty of superior eyelids is performed when redundant skin of superior eyelids prevents normal view, eyes quickly get tired, and face has tired appearance. However, heredity is of great importance, because even good face skin care does not prevent age-specific changes. Frequently, the hypodermic fatty hernias are formed in the area of superior eyelids which are visible at medial angles of eyes. This is the additional indication for blepharoplasty. The following can be referred to blepharoplasty indications:

  • Superior eyelid overhang
  • Skin overhang in the area of lateral angle of eye
  • Evagination of fatty tissue surrounding eyeball in the area of superior and inferior eyelid – “bags” or “fatty hernias”
  • Deep folds in the area of inferior eyelid and at the angles of eyes
  • Wish of patient to change eye form or shape

Blepharoplasty of inferior eyelids is performed upon availability of baggy lower eyelids.

blefaroplastika-3Blepharoplasty essentially has no age restrictions. However, there is a number of blepharoplasty contraindications:

  • Devastating medical diseases
  • Acute and chronic decompensated cardiovascular diseases
  • Contagious diseases
  • Oncological diseases
  • Clotting defect
  • Bronchial asthma
  • Pancreatic diabetes
  • Systemic diseases (rheumatism, lupus erythematosus, etc.)

Blepharoplasty is not recommended for patients with high intraocular pressure and thyroid gland diseases accompanying by its hyperfunction
In addition, blepharoplasty is not performed in case of chronic eyelid diseases and conjunctiva.

  • Superior eylid blepharoplasty
  • Inferior eylid blepharoplasty
  • Transconjunctival inferior eylid blepharoplasty
  • Circular blepharoplasty (single-stage superior and inferior eyelid plastic recnstruction)
  • Asian eylids blepharoplasty (Europeanisation)

During blepharoplasty the surgeons use precision equipment (special magnification optical system) enabling to put regular intradermic cosmetic stitches with microsurgical accuracy.

In addition, when putting the cosmetic stitches there is used special superfine stitch material; therefore, postoperative seams become almost unnoticeable already in 1.5 months.

blefaroplastika-4The first stage of blepharoplasty preparation is the patient preparation. Blepharoplasty implies rather serious intervention in orbital tissue. Therefore, prior to blepharoplasty you should stand full examination. All standard analyses (complete blood count, clinical urine analysis, blood coagulation profile, and blood test for syphilis, human immunodeficiency virus, markers of hepatitis, blood group and Rh factor) are performed in the preoperative period. Sub-studies are conducted upon availability of indications (complete biochemical blood count, ultrasound organs examination, professional advices, etc.). In addition, a patient should:

  • not take any medicaments affecting blood coagulability (for example, aspirin, antitussive) 2 weeks prior to blepharoplasty
  • have “fasting” day (delicate food in small amounts) day before blepharoplasty
  • not eat and drink in the morning before blepharoplasty

Blepharoplasty can be performed at hospital under regional anesthesia or under anesthetic in combination with regional anesthesia. Surgery can last from 30 minutes till 2 o’clock. There is a set great number of blepharoplasty techniques, some of them have become outdated, some are applied to the present day. Modern blepharoplasty techniques assume almost unnoticeable incisions of eyelids located along the natural folds, and maximum preservation of eyelids fatty tissue. They include techniques enabling to perform blepharoplasty without lateral incisions and stitches. Blepharoplasty operation is completed with application of dressing to be removed 4-6 hours after intervention. Pains in the postoperative period are not expressed.

Surgery stages:

  • Marking out of redundant skin
  • Skin incisions
  • Excision of redundant skin excesses and fatty hernias
  • Skin cosmetic stitching
  • Stitch sterile cohesive bandages

blefaroplastika-5Stitches are removed within 3-6 days after blepharoplasty. Generally, you can be discharged from hospital in the evening after surgery or on the next day. Edema of tissues can be observed within 1-2 weeks after blepharoplasty. Bruises usually pass within 5-8 days after surgery. The majority of patients can turn up to work within 7-10 days after blepharoplasty. From the same time you can use decorative hypoallergenic cosmetics. Sun glasses perfectly protect not only against ultraviolet, but also against stranger sights; therefore it is recommended to use them within 1-2 weeks after blepharoplasty. In most cases, eyelid scars are not visible after blepharoplasty, by effect remains forever.

  • Subcutaneous hemorrhages (smoking patients have a higher risk, they will disappear absolutely in 7-10 days)
  • Hypertrophic scarring in the place of cutaneous sutures (occurs in case of skin personal tendency to hypertrophic scarring)

  • You can open your eyes just after surgery; within the first hours chilled gel pads are applied to eyelids in order to reduce postoperative edema
  • Length of hospital stay makes 2-12 hours under the care of duty doctor
  • You should use antiseptic eye drops within 3 days after blepharoplasty for the prophylactic purpose
  • It is advisable to keep indoors within 3-4 days – you should limit the physical load
  • You can take shower even on the next day after surgery considering that within 3-4 days after surgery it is prohibited to touch eyelids during washing
  • Sutures are removed on the third day
  • Edema of periorbital regions intensifies by 3-4 day after surgery, and then absolutely disappears within 1-1,5 weeks
  • You can do your face within 7-10 days after blepharoplasty
  • Scars become unnoticeable in 2-4 weeks

A patient can estimate final surgery effect in 1.5 months when a seam will completely be formed and postoperative edema will disappear. As practice shows, the second correction may be required not earlier, than in 7-10 years after the first blepharoplasty.

If patient wishes we can perform superior eyelids operation several times, inferior eyelids re-interventions can be made on medical indications only. Certainly, duration of effect in many respects depends on state of patient’s health, availability of social habits and way of life. Therapeutic programs of the cosmetician: face lymph drainage massage, simulator, moisturizing sets, lifting-programs and special soft pilling facilitate a postoperative rehabilitation period and prolong surgery effects. Fine lines at lateral angle of eyes (“crow’s feet”) are easily removed through the injections of botulism neurotoxin (“Disport”, “Botox”) or bio-gels (“Restylane”). You can administer specified medicines in 3 weeks after blepharoplasty. Dermatocosmetologists together with cosmetic surgeons will fit individual total care for domestic purposes that will help to improve significantly a skin condition around eyes, to keep the reached aesthetic effect for a long time.

3-6 weeks are required for estimation of the effect. Over this period the flexibility of tissues is restored and scars are resolved. Generally, effects of blepharoplasty are longest in comparison with other aesthetic surgery operations. Eyelids surgery enables to correct looseness of skin tissues and to eliminate fatty hernias, correcting consequently weary and aged sight. Actually, full and final removal of baggy lower eyelids is observed.

On the contrary, skin continues to age that leads to folding. Necessity for the subsequent surgery occurs, mainly, not earlier than in 12 years. Post-blepharoplasty effect remains forever. In most postoperative eyelid scars are not visible, eyelids plastic reconstruction forms young, vigorous and rested face appearance.
