WHAT IS OTOPLASTY?

Otoplasty (Greek Othos – ear) – the general term for designation of plastic surgeries on ear auricles.

Otoplasty is the surgery on the improvement of the form and sizes of ear auricles, removal of everted ears.

The plasty of ears helps to reduce the ears, to remove the protruding ears (everted ears), congenital and post-traumatic defects of ear auricle, to restore the ear auricle in their absence.

Ear plasty supposes surgery which touches soft cartilaginous tissue of ear auricle. There are many options of ear deformation. For each correction there are own methods. Patented effective methods help to pass the surgery easily and without complications.

Most often ear plasty (otoplasty) is made in order to clamp down the protruding ears closer to the head or to reduce the size of large ear auricles. INDICATIONS FOR OTOPLASTY

The main indications for otoplasty include:

  • incorrect form of ear auricles
  • everted ears
  • smooth anterior surface of ear auricle and antihelix
  • increasing the angle between the occipital lobe and ear auricle, partial or complete absence of ear auricle (congenital or evoked nature)

CONTRAINDICATIONS FOR OTOPLASTY

  • It is prohibited to make otoplasty to the child if he/she is not interested in it – that is to say the changing in the appearance should be wanted by the child, not by the parents. Ear plasty is contraindicated in the acure catarrhal disease and chronic ear infection. Otoplasty should not be made when the patient has any acute disease. Otoplasty is also contraindicated in:

WHAT KINDS OF SURGERY ARE THERE?

otoplastika-03Otoplasty may be esthetic (the form and sizes of ear auricle are changed) and reconstructive (restorative) which is made in the absence of the part or entire ear auricle.

Esthetic otoplasty is more often made in order to clamp down the protruding ears closer to the head or to reduce the size of large ear auricles. The reason for otoplasty is often the two-pronged ear lobe as the congenital defect or the result of trauma from wearing heavy earrings. The form of ears is very often the result of psychological trauma especially in the growing age when the increased attention is paid to the appearance. So, if your child has incorrect form of ears otoplasty should be done at the age of 5-6 years.

Reconstructive otoplasty allows completely or partially to restore the ear auricle. It is more complicated type of cosmetic surgery requiring high qualification of the surgeon. The indication for reconstructive otoplasty is the absence of ear auricle or its parts. The surgery is made stage by stage within the year. The first stage is the creation of cartilage matrix for ear auricle (usually from the patient’s rib). The second stage – the formation of the subcutaneous pocket at the place of absent ear and placement of cartilage matrix there which settles down within 2-6 montha, then it is finally fixed in correct position – it is the third stage of otoplasty. At the fourth stage of otoplasty the required notches and hircus are formed.

WHAT IS REQUIRED FOR SURGERY?

First, the contraindication for otoplasty are severe somatic diseases. Women are not recommended for otoplasty during menstuation bleeding. HOW IS SURGERY PERFORMED?

otoplastika-05The correction of everted ears or other forms of ear auricle disorders includes the creation of new form of ear cartilage. On the posterior surface small discission is made, then otoplasty provides for the changing the ear cartilage form that is the cartilage tissue in otoplast shall be moved to the optimal position. New form shall be mandatorily fixed with the help of surgical sutures. The whole surgery takes 3-40 minutes and with the hands of high quality professionals the otoplasty shall allow to acquire the required form of ear auricle of the patient. Wounds heal rapidly and the edema after surgery is minimal, the tissues are less traumatized, suffusion is not observed, hence the rehabilitation period is the minimum.

Usually, after otoplasty by laser method the bandage is removed in 6 days. Otoplasy is not considered difficult surgery however, in post-operative period it is required to wear special bandage. Also, in pains and discomfort after otoplasty the analgesics are recommended.

The doctor always tells that the correction done does not render effect on hearing. THREE METHODS ARE USED MORE FREQUENTLY:

1. Otoplasty is described by Furnas. Behind the ear the plot of skin is dissected between the ear auricle and the skull. Cartilage is stitched to the temporal bone and thus the ear is pressed to the head.

Two other methods are based on the liquidation of everted ears due to formation of ear cartilage fold (anthelix).
2. Mustard proposed to make long discission along the posterior surface afther that the cartilage becomes thinner along the future fold and sutures are ut on it. These sutures bend the cartilage, forms the fold of the anthelix and, thus, gives the required form of the ear and eliminate the everted ears.

3. Etenstrom developed the surgery when at the posterior surface of the ear the discission is made with the length of ̴ 1 cm, through this wound the cartilage is thinned under the skin along the anterior surface. Then, through t he small discissions (2-3 cm) on the posterior ear surface 2-3 sutures are put which gives the required form of the ear. Since in 2000 in the clinic Rechts der Isar in Munich I has first mastered this method I became less to use other methods.

In all cases otoplasty is completed with the application of sterile gauze wads on the ear. Then the tennis band for hair is put on. The first dressing shall be the next day. Sutures are removed on the 7th day.

SOME NUANCES OF POST-OPERATIVE PERIOD

It is better to exclude physical exercises for the next three weeks after otoplasty. In six months the healing of the ear shall complete. Most of the patients return to their daily activities in 3-5 days after esthetic otoplasty. After otoplasty for children the ears grow together with the child growth.

Esthetic otoplasty is less traumatic surgery for the patient but it requires the high professionality from cosmetic surgeon both in medical and artistic meaning.

RESTORATION AFTER OTOPLASTY

Generally, otoplasty (ear correction) does not require staying in the clinic. In the first days after otoplasty there is post-surgery swelling in the area of surgical wound, slight pain can be observed which can be relieved with general analgesic preparations.

Within two weeks after otoplasty (ear correction regular dressing should be made, approximately, 1 time every 2-3 days. At the same time the doctor controls the healing process. More often in the discission behind the ear auricle the suture is made with the special dissolvable stitches and these sutures are not removed. In case when the sutures should be removed they are removed on 8-10th day. Within a week after otoplasty it is necessary to wear bandage fixing the correct position of ear auricles. In several days the person can return to usual life. The result is saved for the whole life.

RESULTS OF OTOPLASTY

Otoplasty generally does not have complications and significantly improves the appearance. Otoplasty is considered to be easily tolerable surgery and in many cases otoplasty shall bring perfect results and help to make away with annoying disadvantage.
